Choosing the Right Time for Glaucoma Intervention

Glaucoma treatment often begins before a patient notices any change in vision. The appropriate time to consider an interventional treatment depends on the patient’s eye pressure, optic nerve health, glaucoma progression, and ability to maintain a daily medication routine.
Why Treatment May Be Needed Without Symptoms
Glaucoma gradually damages the optic nerve, which carries visual information from the eye to the brain. Although increased eye pressure is a major risk factor, glaucoma can also occur at pressures that fall within a range considered normal.
Most patients do not experience pain or early warning signs. Peripheral vision may decline slowly, making the change difficult to recognize. For this reason, glaucoma treatment decisions are based on examination findings and diagnostic testing rather than symptoms alone.
Regular testing allows an ophthalmologist to compare results over time and determine whether the disease is stable or progressing.
Considering More Than Daily Eye Drops
Prescription eye drops remain an effective option for controlling eye pressure. However, they must be used correctly and consistently.
Some patients struggle to remember every dose or have difficulty placing drops in their eyes. Others experience irritation, medication costs, or complicated schedules involving several prescriptions.
These challenges do not mean that glaucoma cannot be controlled. Interventional treatments may provide another way to lower pressure while reducing dependence on a daily medication routine.
When SLT May Be Considered
Selective Laser Trabeculoplasty, commonly called SLT, is an office based laser treatment that helps improve the eye’s natural drainage system. Better drainage allows fluid to leave the eye more efficiently, which may lower pressure.
Potential benefits of SLT include:
- No surgical incisions or stitches
- Minimal discomfort and recovery time
- Reduced reliance on medication for some patients
- The possibility of repeating treatment when appropriate
SLT may be considered as an initial treatment for certain patients with open angle glaucoma or ocular hypertension. It may also be recommended when medication is difficult to manage or is not providing enough pressure reduction.
Results vary, and some patients will continue using glaucoma drops following the procedure.
When MIGS May Be Appropriate
Minimally Invasive Glaucoma Surgery, or MIGS, refers to several procedures designed to lower eye pressure with less disruption than traditional glaucoma surgery.
Depending on the procedure, MIGS may improve the eye’s natural drainage system, create an additional pathway for fluid, or reduce fluid production. Potential advantages may include small incisions, a relatively brief recovery, and less reliance on medication for some patients.
MIGS is often considered for eligible patients who are also undergoing cataract surgery. This creates an opportunity to address cataracts and glaucoma during the same surgical experience.
However, MIGS is not the right choice for every patient. More advanced glaucoma may require a different surgical treatment to reach a sufficiently low eye pressure.
